I, a parent or legal guardian of the Program Participant, hereby grant Circuit Stream, a perpetual, worldwide, non-exclusive, fee and royalty-free, sublicensable right and licence to:

• (a) Broadcast, directly or through an agent, by any means, the learner participating in the Programs (the “Program Participant”) appearance or participation in the Programs offered by Circuit Stream (the “Program”);

• (b) Record, directly or through an agent, the Program Participants appearance or participation on video, audio tape, film, photograph or any medium, now existing or hereafter developed (“Recordings”);

• (c) Use, reproduce, modify, distribute and publicly display, in any manner, any written materials or presentations distributed, shown, or otherwise provided or used by the Program Participant in connection with appearance or participation in the Program for any purposes, including, without limitation, commercial purposes; and

• (d) Use the Program Participants name, likeness, voice and information in connection with the promotion of the project or event and the exercise of the rights granted to Circuit Stream above.

I agree that I have no right of approval, no claim for compensation, and no claim arising out of or in connection with any use of the Images/Recordings or Materials as provided above. For the avoidance of doubt, I agree that Circuit Stream may make any such broadcast, Recordings, or Materials available to the public.

Confirmation

I represent that I have read the foregoing, fully understand its meaning and effect, and have all rights necessary to grant the rights and licences granted in this Release and License and that the exercise of the rights and licences by Circuit Stream will not infringe on or violate the rights of any third parties. I also understand that if I choose not to sign, this will have no impact on the Program Participant’s participation in the Programs offered by Circuit Stream.
